My personal goal is to raise at least $3,000 for the Whitman-Walker Clinic in Washington, DC, before December 1, 2003. To reach this goal, I would like to ask for your help.

1. If you want to use the online credit card donation option, please go to the secure AIDS Marathon website and follow the steps there. The link will take you directly to my AIDS Marathon Donation Page. This will save you trouble writing a check and then mailing it.

2. If you prefer sending a check, you can download the donor form (291 KB, Adobe Acrobat format), fill it in, enclose the check and then mail it to the AIDS Marathon Program. Please don't forget to fill in my name (Marek Filicko) and my participant number (3096) at the top. I can also send you the donor form by mail, including the stamped return envelope, so the only thing you would need to do would be to enclose your check and take the envelope to the post office. For this option, send me an e-mail at marek-dot-filicko-at-atlas-dot-cz with an address to which I should send the form and the return envelope.

Few notes:

• You can also spread your donation over several months.
• Many companies will match employee contributions. Please check with your employer for specific guidelines.
• Every donation is fully tax-deductible.
• For donations over $250, Whitman-Walker Clinic will send you written acknowledgement.
